Tier System
TIER 1 – OUTSTANDING
TIER 2 – VERY GOOD
TIER 3 – AVERAGE
TIER 4 – BELOW AVERAGE
TIER 5 – POOR
The following areas will be used by Dr. DeLuke and his Staff to determine which Tier a patient will be placed into:
- Oral Hygiene
- Compliance with prescribed Elastic/Appliance wear
- Breakage of Appliance(s)
- On Time Arrival to appointments
- Number of Missed and/or Rescheduled Appointments
At the end of treatment the patient will be given a final Tier Level assignment and awarded the corresponding number of points. A patient can move up or down in Tier Levels throughout treatment. The Tier Level assigned to a patient is not subjective, as it is based on his/her performance throughout treatment.
PLEASE NOTE: If you were collecting tokens under the old system, those tokens do not “transfer” to a Tier level. But don’t worry, if you were doing well under the old system you should have no problem reaching a high Tier level and getting the prize(s) of your choice at the end of treatment!
